The following is a list of AHSHA forms that are used by players, parents and team staff throughout the season:
If you are going to be on the ice, bench, or interact with the players, you need to register with USA Hockey. Remember, you must have a current registration for the entire season you are applying. A current USA Hockey registration number is required to complete this application USA Hockey

- Applications: If you would like to become involved within AHSHA the first step is to complete one of the following APPLICATIONS:
